ABOUT US


Care Management Solutions (CMS) offers professional assistance to help older people and their families find solutions to the challenges of aging. 

A CMS Care Manager (aka a Geriatric Care Manager) is a health professional who will answer questions and coordinate services and care needs including in-home care services, community-based services, and public or private service options. 

CMS can research service eligibility, costs for the client/family, and assist the client to access or interview potential service providers or home care staff. CMS can oversee the supervision of these services once in place, as an advocate for the client and family.

Whether family live next door or far away, or if a client has no immediate family, Care Management Solutions can be there to help.   CMS can act as a local support system, make home visits with the client, supervise care service providers, and submit written reports to the family. The CMS Care Manager is available and involved with the client/family as much or little as they decide is needed.

SERVICES

SERVICE NEEDS ASSESSMENT

At an initial in-person meeting with the client and/or family the CMS Care Manager will:

  • Identify the goals and wishes of the client.

  • Identify any gaps in care or services essential to meeting those goals.

  • Develop a plan of care to manage the complex and changing needs of each client to maximize their best quality of life possible.

HOME CARE MANAGEMENT

Screen, coordinate, schedule, and monitor in-home supportive care staff and other services (certified or private pay).

Provide information, site visits, and assistance in investigating alternate living options such as Independent, Assisted Living, or Continuing Care Retirement Communities (CCRC), if desired.

Assess and arrange for in-home healthcare (licensed/medical) services when needed.

Medication Management: Prefill assistance by a Registered Nurse, physician coordination.

Access local Elder Service Organizations (ASAPs), Councils on Aging, or Adult Day Health program options, as desired.​​

Coordinate other community benefits and services, as needed.

CARE COORDINATION

Develop a comprehensive plan of care.

Coordinate quality resources and make referrals for needed services, agencies, medical professionals, or medical equipment companies.

Coordinate non-medical social support services: friendly visitor programs, transportation options, volunteer services, and senior center activities.

LONG-DISTANCE CARE SUPERVISION

When family does not live next door, or even in the next state,​​ CMS can make regular visits to the clients home, assisted living, or other community, wherever home is for the client.  At these visits the Care Manager will evaluate the client's status, their ongoing care needs, satisfaction with services in place, and assess any changes in condition, safety issues, or other issues necessitating feedback or intervention by the family.

A CMS Care Manager can attend scheduled doctor appointments as an extra set of eyes & ears to note changes, ensure follow-up, and maintain communication between the client and family.

Advocacy in hospitals and long-term care settings. 


Coordination and communication of health care services between the client, family, and health care providers.

There is no cost for the initial assessment call or visit, unless ongoing services are agreed to.

WHO WE ARE

Daniel Dougherty is the Founder and Director of Care Management Solutions. 

Daniel is a Registered Nurse with a degree in Health Care Administration, with more than 25 years of experience in the healthcare field, as both a clinician and administrator.

Daniel has formerly worked as the Executive Director for both Private Home Care and Certified (Medicare/Medicaid) Home Care organizations. He has also held leadership roles in community health centers, adult day health programming, and other community-based elder service organizations. 

Daniel is a member of the Multicultural Coalition on Aging, a committee member for FriendshipWorks, and on the advisory board for the Elders Living at Home Project at Boston Medical Center.
